Practice Setting: Inpatient and Outpatient  
Reason for Coverage: Coverage while we recruit for full-time Oncologists  
Dates of Coverage: asap upon credentialing  
Patient Population for Required Coverage: Adult  
Number of Physicians in Practice: 3  
Will There be Radiation Oncologists Present During Locum Tenens Coverage? yes we have 4 full-time Radiation Oncologists  
How Many APPs Provide Coverage: 5 full-time APs  
Clinical Area: Hematology - Outpatient Setting, Oncology - Outpatient Setting, Oncology - Inpatient Setting, Hematology - Inpatient Setting  
Trauma Level/Designation (Hospital):  
Documentation System/EMR: Epic  
Estimated Credentialing Timeframe: 1 month  
Submission Requirements Board Certified- Required
