Summary: This request is for (1) Diagnostic Radiologist to be onsite during weekdays to readdiagnostic radiology exams as well as perform Fluroscopy exams. There would also be a call need afterhours on weeknights as well as on the weekends (Sat and Sun).

Malpractice
Must be included in rates.

Orientation
***-280 - half the hourly/callback rate*** kicks in after 8 hours. Per hour of
orientation or training beyond 8 hours

Hourly ***-*** - Per hour, not OT

Vacation ***-840 - Must be 1.5x the hourly rate*** Per vacation hour, no OT

Nightly Call
***-*** - Per night call to hold the pager, to include 2.5 hours gratis
