RATE CAPS FOR SUBMISSION:

Malpractice Must be included in rates.

Orientation *** - half the hourly/callback rate*** kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

Daily ***-*** - Per 24 hours daily''

Vacation Daily Call - ***-5250 - 1.5x per 24 hours daily''

Schedule/Availability Requirements:Schedule: Monday-Friday 8a-5p with weeknight call and 24 hour weekend call 1 week per monthShifts per month: Approximately 10 Start date: As soon as credentialed
