Works with program psychiatrist in assessing medical/psychiatric needs of program patients. Provides linkage and liaison with primary care provider. Provides health evaluation and follow-up to referred patients, management and administration of medication under supervision of MD. Evaluates routine urgent health problems and facilitate, coordinate referrals, evaluate laboratory reports. Provides health mental education, treatment, rehabilitation and support services to referred patients. Services may be provided in the community as needed. ...
