Summary This position is located in the Primary Care located at the VA Southern Oregon Healthcare System, White City VA Medical Center, White City OR. Responsibilities The full performance level of this vacancy is GS-6. The actual grade at which an applicant may be selected for this vacancy is in the range of 3 to 6. Duties of this position include but are not limited to the following: Provides ongoing, continuous care of one panel of Veterans. Ensures planning, assessment and appropriate evaluation and access is provided to Veterans assigned to the PACT panel. Participates in team performance improvement and sustainment activities to optimize team efficiency and care delivery to Veterans. Manages communications and facilitates safe transitions of Veterans between the PACT's site of care and other health care settings, using informal and formal communication methods as appropriate.
