LPN Position in Grafton County Region

This position covers the northern Grafton County region (Plymouth, Benton, Wentworth, Pike, Campton, Piermont and Haverhill areas).

Job Summary:

As an LPN at NCHHHA, you will have the ability to get to know your individual patients and support your team members in executing tailored plans of care for Home Health and Hospice patients. You will perform comprehensive assessments and provide skilled nursing care for each patient under the supervision of and in partnership with a qualified Registered Nurse. Our LPNs are an integral part of a dynamic, multi-disciplinary team. You will maintain a high level of autonomy and work to the top of your license. You will enjoy a flexible workday, with an option to start and end your day from home.
