ResponsibilitiesPOSITION SUMMARY: The Lead Case Manager responsible for managing resource utilization and clinical outcomes for patients, as well discharge needs of the patients. He/She will also be responsible to assist with the denial management process.
QUALIFICATIONS:
1. Bilingual preferred (Spanish)
2. Must have at least two collective years of case management or social work experience, with a high
preference for multi-unit exposure. 5 years experience preferred.
3. Must demonstrate commitment and adherence to STHS’s Compliance Program and Code of Conduct through compliance with all policies and procedures, the Code of Conduct, attendance at required training and immediately reporting suspected compliance issue(s) to the Compliance Officer.
