About the Role: We re seeking Advanced Practice Providers to join a nationally recognized leader in in-home care for patients with chronic kidney disease and end-stage renal disease. This innovative model combines technology-enabled care management, medication review, and patient education to improve outcomes, reduce costs, and expand access to high-quality, evidence-based care.
A Day in the Life: - Conduct comprehensive initial assessments, follow-ups, and hospital transitions
- Perform hands-on, head-to-toe assessments and obtain full vitals
- Manage medications and provide disease process education
- See approximately 4 5 patients per day
- Visit durations: 90 minutes for new patients, minutes for follow-ups
