Notes: PT must have 2 years of experience providing physical therapy services in hospital or inpatient clinical setting. Physical Therapists shall possess a Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from a program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE)
