Hospital-Based Hospice Nurse Liaison

Named by the Boston Globe as one of the Top Places to Work in Massachusetts for the last four years, our community based, mission-driven hospice and palliative care organization seeks a Hospital-Based Hospice Nurse Liaison to join its expanding Intake and Admissions team. This is an on-site position located at Newton-Wellesley Hospital .



Job Responsibilities :

The position of Hospital-Based Hospice Nurse Liaison (HBHNL) is to provide information about hospice services to patients, families, and hospital-based staff, to conduct clinical assessments, coordinate hospital discharge plans, and maintain professional relationships within hospitals, with a focus on the assigned hospital.

Monday through Friday from 9 am to 5 pm (40 hours)
