**Hospice RN Case Manager (RN)**

**Location: Waterbury Region**

**Status: Mon – Fri 8 am to 5 pm, occasional backup - holiday/weekend**

**Bonus: $10,000 sign on bonus**

As a Home Health Registered Nurse, you’ll deliver skilled nursing care in the comfort of patients’ homes, creating individualized care plans, administering medications, educating patients and families and supporting recovery and independence.
