Position Overview

The Hospice RN Case Manager provides skilled nursing and case management services to hospice patients, focusing on ongoing care coordination rather than admissions. This role supports patients and families through comprehensive hospice management, interdisciplinary collaboration, and compassionate end of life care.

Schedule & Work Details

• Coverage Area: Lewis County


• Schedule: Monday through Friday, 8:00 AM to 4:30 PM


• Weekly Hour Guarantee: 40 hours


• On Call/Weekend Requirement: On an as needed basis when on call or weekend staff call out


• Mileage Reimbursement: Yes, reimbursed between patient visits from first patient to last patient


• Pay Rate: $51 to $53 per hour
