**Description**



This part time (24 hours/week) position will be supporting patients in Jackson and Josephine counties.



Under the supervision of a registered nurse, provides supportive personal care services and homemaking assistance to patients of all ages according to the individualized physical needs of the patient in accordance with state and federal regulatory requirements.
