As a **Home Health Account Executive** , you will:

· Call on physicians, hospitals, SNF management, discharge planners, and case managers within an assigned territory to promote our home health care services; building and maintaining referral source relationships.

· Prepare business plans and maintain target lists. Prioritize accounts following the market sales plan.

· Gather and organize account-related information and provide input on important customer opportunities.

· You will report to a Market Executive of Sales.

****Please note the territory includes McHenry and a small portion of Lake and Kane counties**
