Responsibilities Two Full time position exist for an experienced registered nurse in the Geriatric Extended Care (GEC) Home Based Primary Care (HBPC) Program This position reports to the Nurse Manager of Home-Based Primary Care. The HBPC RN is a member of the interdisciplinary team providing direct primary care to a specific group of homebound patients with a variety of chronic and terminal illnesses.
