Home Based Primary Care Nurse Practitioner

This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until July 10, 2026. The initial cut-off date for referral of eligible applications will be May 25, 2026 with subsequent cut-off dates every 2 weeks. Eligible applications received after that date will be accepted on an ongoing basis and qualified candidates will be considered as vacancies become available. The Home Based Primary Care Nurse Practitioner (HBPCNP) serves veterans with complex, chronic, and disabling diseases who have difficulty traveling to VA facilities for care. HBPC delivers comprehensive, interdisciplinary primary care right in the veteran's home. Given the rural and geographically isolated setting of Eureka, the HBPCNP plays a critical role in ensuring access to timely, continuous primary care services.
