HMO

• Opens the Call
• Protects and secures member’s Protected Health Information (PHI).
• Reviews HRA and completes Mode of Care
• Based on identified needs, creates an ICP (Identified Care Plan).
• Identify AOF (Area of Focus) and come up with active interventions based on AOF.
• Completes health teaching.
• Documents in the client system.
• Closes the call

Education : Bachelor’s degree in allied sciences, Social Sciences, Social Work, Human Services, or a related field.

Experience :

• Minimum of 1-year experience in Direct Patient Care, such as in Long-Term Care, Home and Community-Based Services, Hospital/Facility Settings, Or Case Management.
