Overview:
We are hiring 10 consultants for a special project supporting the Campus Support Center in Glendale, CA. This team will assist 8 full-time employees in delivering high-quality patient outreach and care coordination.
Responsibilities:

+ Make outbound calls to patients for:

+ Pharmacy refill confirmations

+ Annual wellness visit scheduling

+ ER outreach for frequent emergency room visitors

+ New authorizations and other leadership-driven initiatives


+ Accurately enter and manage data in Excel and internal systems

+ Perform insurance and benefit verifications

+ Coordinate care and follow-up tasks as needed
