Nurse Practitioner - Value-Based Primary Care in Ontario, CA making $150k

Compensation:

• Base: $140,000+ (Flexible based on experience)
• Bonus: Up to $10,000 annually
• Sign-On Bonus Available

Schedule

• Monday-Friday, 8am-5pm
• No nights, no weekends
• Limited call: 1 week, 3-4x per year
  • First call handled by call center
  • NP is rarely contacted after triage

Practice Model

• Value-Based Outpatient Primary Care
• Geriatric population
• Small patient panel
• 12-16 patients/day
• Dedicated Care Team:
  • MA, Scheduler, Referral Specialist, Social Worker, Pharmacist
