Job Summary

Geisinger is seeking a Family Nurse Practitioner (FNP) to join our team supporting care for adults and adolescents based at the Geisinger Behavioral Health Center in Moosic, PA. The FNP will work collaboratively within a multidisciplinary team to deliver high quality, patient centered care.
Job Duties
+ Perform comprehensive medical assessments.
+ Provide medication management, evaluations, and daily patient rounding.
+ Develop and implement treatment plans in collaboration with psychiatrists, psychologists, nurses, therapists, and case managers.
+ Document all care in the Epic electronic health record.
+ Participate in treatment team meetings and coordinate care transitions.
+ Communicate with families, caregivers, and community providers as needed to support continuity of care.



Practice Environment
+ Patient population: 75% adult, 25% pediatric (age 14+)
+ Daily encounters: 10–16
