An exciting opportunity has arisen to join the Leeds Community Diabetes Service as a Band 4 Facilitator.

We are looking for a motivated, organised, and compassionate individual to support the delivery of high‑quality diabetes care across the community.

This is a permanent and full‑time position. Working as part of a multidisciplinary team, you will play a key role in enabling patients to better manage their diabetes through education, engagement, and personalised support.

You will contribute to the coordination and facilitation of structured education programmes, clinics, and community‑based initiatives aimed at improving health outcomes and reducing health inequalities across Leeds.
