Work Schedule: Full Time, 72.00 hours per pay period (2 weeks)

Pay: $33.82 - $50.73 / hour. Pay is dependent on applicant's relevant experience

This position is an onsite role and does not offer a hybrid or remote option

Summary:
Coordinates the care of patients with chronic respiratory disease.

Responsibilities:


+ Coordinates patient assessment and care, including collaborative care, case management, education, and discharge.

+ Participates in multi-disciplinary meetings for the purpose of managing/reviewing patient care and treatment protocols, policies, and procedures.

+ Serves as a resource to staff in areas of program development, clinical practice, and professional development.
