Community Mental Health Nurse - MHSOP

We are looking for a community mental health nurse to provide holistic assessment and treatment pathways for older adults with mental health and frailty needs. The role focuses on seamless pathways between primary and secondary care for functional mental illness and dementia care.

As part of a multi‑disciplinary team, the nurse will work alongside Clinical Psychologists, Consultant Psychiatrists, Occupational Therapists, Social Workers and other Mental Health Nurses, and will participate in CPD, reflective practice, supervisions and academic training.

The post holder is required to take responsibility for the assessment of care needs, development, implementation and evaluation of programmes of care, setting standards of care and managing a defined caseload.
