About the Role
Your role will involve providing assessments, support and education visits in the community. You will assist people and those supporting them to manage their conditions in their normal environments including aged residential facilities.
Responsibilities
- Provide assessments, support and education visits in community settings for older persons with mental health needs.
- Provide education on specialist topics both formally and informally to health professionals and community members.
- Work alongside a multidisciplinary team consisting of specialist nurses, health care assistants, occupational therapists, physiotherapists, social workers, psychiatrists, and psychologists.
- Assist in managing complex mental and physical health needs of clients.
