We are seeking a dedicated and compassionate Community Learning Disabilities Nurse to join our multidisciplinary team. Responsibilities

• Provide specialist nursing care and health facilitation for individuals with learning disabilities.
• Undertake comprehensive health assessments and develop person‑centred care plans.
• Work collaboratively with families, carers, and other professionals across health and social care.
• Deliver health promotion, education, and training to service users, carers, and staff.
• Provide specialist support, advice and guidance to individuals, families and carers.
• Provide specialist support as part of the multi‑disciplinary team.
• Complete specialist assessments as clinically indicated.
