Overview:

Sentara Medical Group is seeking to hire an individual to join our team as a Community Health Worker

Position Location: Harrisonburg, VA (rotation between clinic 8-5 pm and mobile bus)

Position Status: Full-time Days

Standard Working Hours: Monday - Thursday, 10:00 am -7:00 pm and Fridays, 8-5 pm (varies on mobile bus)

Night or weekend flexibility is needed for occasional community events.

Works in collaboration with the Case Manager and other healthcare team members to provide comprehensive and coordinated care. Engages in interventions to improve self-management skills and improve quality of life for patients with chronic and/or complex illness.
