Position Summary

As an integral member of the Coding and Abstracting Department, the Health Information Management (HIM) Professional – DAD Coder utilizes their technical, analytical, and decision‑making skills to interpret ambulatory clinical documentation. They apply knowledge of medical and surgical terminology, anatomy, pathology, physiology, pharmacology, and national and provincial coding standards.

Duties

• Inpatient Coding: Interpret patient experiences from dictated and written clinical documentation for ambulatory visits, coding and abstracting mandatory health data to complete DAD inpatient abstracts per CIHI guidelines, Ontario MOHLTC directives, and UHN initiatives. Enter information via an electronic abstracting system.
• Diagnosis: Identify all relevant conditions described by physicians or other health providers during the ambulatory visit, using ICD‑10‑CA.
