Position Summary:      

• The pharmacist will be responsible for medication therapy management (MTM) services, including outbound calls for comprehensive medication reviews (CMR) and delivering in-depth consultations on statin medication use per AHA guidelines. The pharmacist will transcribe patient-specific prescription information, diagnosis information, and recommendations into a third-party, web-based application.
