Clinical Lead Referral and Assessment Hub

We are continuing to develop our referral and assessment hub and are looking to recruit a B7 Clinical Lead to support referrals, assessments and clinical leadership within the hub.

Referrals will be triaged by a member of the team, and a decision will be made on the onward journey for that service user.

You may provide assessments for the team and will complete those assessments at that designated base or at a venue agreed with the service user and carer.

This post is a full time position (37.5 hours per week) and the working hours will be 9am - 5pm 5 days per week. The post is based at Central Park Hub although the post holder will be expected to travel across the Borough to attend satellite clinics.

This post includes participation in the Bronze on call rota.
