**Job Summary and Responsibilities**

We are seeking a skilled and compassionate Chaplain to join CommonSpirit Health at Home. The Chaplain will serve as an integral part of the multidisciplinary team providing spiritual and emotional support for patients, caregivers and staff.

+ Participates in interdisciplinary care conferences to assist in assessing the spiritual and emotional needs of patients and caregivers including psychosocial issues, adjustment to illness or disability, status of religious/spiritual coping, supporting others in the process of identifying and applying their own values in decision making.
+ Demonstrates sensitivity to the needs, customs, or feelings of patients and caregivers.
+ Makes home visits to attend to patients at the end of life.
+ Provides support and comfort to the dying patient and caregivers, as well as prayer when requested by patient or caregiver.
