**Description**



Assesses needs and provides Certified Occupational Therapy Assistant (COTA) services and direct treatment in consultation with the Occupational Therapist. The COTA functions as a member of the Interdisciplinary Team. Responsible for home care coordination.

**Required Qualifications:**



+ Graduate of a Certified Occupational Therapy Assistant program.

+ Within 30 days of hire: National Provider BLS - American Heart Association

+ Upon hire: Oregon Occupational Therapist Assistant License
