Description

JOB SUMMARY The Case Manager’s main responsibility is to provide case management services for a caseload of 22 clients and their family members. The Case Manager prepares comprehensive assessments and service plans. Home visits and Field work are required.

ESSENTIAL FUNCTIONS AND KEY ACCOUNTABILITIES


+ Ensure that each client is receiving appropriate and effective case management services

+ Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goals.

+ Conduct face to face contacts (home visits/office visits) as specified in the service plan(s); with a minimum of one home visit per month and one contact per week.
