The Care Management Support Coordinator provides administrative and telephonic support to care management teams. This role focuses on member outreach, inbound call handling, scheduling services, and assisting members in navigating care plans and available resources. The coordinator serves as a key point of contact for members, providers, and internal teams, ensuring high-quality service and compliance with regulatory standards.
Key Responsibilities

+ Conduct outbound calls to members to support care plan follow-ups, scheduling, and ongoing education

+ Handle inbound calls from members and providers, addressing inquiries, concerns, and service-related questions

+ Assist members in accessing health plan benefits, community resources, and support services

+ Provide guidance on care plan procedures, protocols, and next steps
