Location: Westbury, NY | Schedule: 35 hours per week | Status: Non-Exempt |Salary:20.61/Hour

The Opportunity

The Young Adult Recovery Care Coordinator provides comprehensive care coordination and service navigation for young adults ages 18-24 residing in recovery residences across Long Island. This role addresses Social Determinants of Health (SDoH) needs, connects participants to essential clinical, social, and vocational resources, and ensures continuity of care across systems. Working closely with the Young Adult Residences Supervisor and Recovery Coaches, the Care Coordinator supports participants in achieving stability, wellness, and sustained recovery.



Responsibilities


+ Conduct comprehensive, strengths-based assessments, including identification of Social Determinants of Health (SDoH) needs, and collaborate with participants to develop and update individualized service plans.
