Job Description & Requirements Job Scope A. Programme Development and Implementation i. Assist in running of targeted outreach and cognitive stimulation activities. ii. Support in initial assessment of walk-ins and referral cares, follow-up intervention plans for clients as per the prescribed care plan and under the guidance of Allied Health Professional iii. Support in the monitoring the client's condidtions and progress and escalate to Allied Health Professional where necessary iv. Plan and conduct outreach to the community and stakeholders to promote mental wellness and mental health awareness, including community mapping v. Assist in executive of structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.
