**Job Summary:**

Centennial Heart Cardiovascular Consultants at TriStar Centennial Medical Center is seeking an experienced advanced heart failure advanced practice provider to join their group in Nashville, Tennessee.

**Qualified Candidates:**

· Nurse practitioner or physician assistant certification

· Tennessee licensure or ability to obtain

· Advanced heart failure experience preferred

· Ability to work collaboratively supporting 5 advanced heart failure physicians

· Full time, M-F, outpatient only
