Job Description

Provides direct services to individuals regarding concrete services and entitlements; assists with some aspects of discharge planning. Provides information to both 'walk-in' patients and telephone inquiries. Oversees special workshops and socialization groups.

Job Responsibility

+ Assists care managers with cases requiring emergency intervention; discusses, with the social worker, care plan and means to facilitate patients' goals.
+ Identifies unmet biopsychosocial needs and under the supervision of coordinator; assists patients to meet those needs effectively.
+ Maintains documentation in the patient chart in accordance with department requirements; prepares and submits reports as required.
+ Maintains related records and charts progress notes; documents the nature and extent of interventions made; makes copy of record for the Unit file.
