The incumbent will provide active ageing activities, befriending services for vulnerable seniors who require additional social support; and referrals for care services when required. He or she works closely with the Centre Supervisor to promote healthy living and keep seniors engaged within the community which includes regular monitoring of seniors' health condition.

• Assist Centre Supervisor to conduct active ageing activities and organize special events (i.e. National Day, Christmas, Chinese New Year) for the seniors and their caregivers.
• Conduct community outreach and needs assessments to seniors at a designated service boundary.
• Provide befriending services to vulnerable seniors including connecting them to care services when necessary.
