Cleveland Clinic's Heart, Vascular and Thoracic Institute's world-renowned Heart Transplant team has a full- time opening for an experienced Advanced Practice Provider (NP/PA).

The ideal candidate will be energetic, motivated, team oriented, possess excellent verbal and written communication skills, and provide exceptional care to patients. The patient population served by the advanced practice provider will be post heart transplant patients. This provider will triage patients, practice in both the inpatient and outpatient setting in which patients require complex monitoring and therapies, high intensity intervention, and continuous vigilance within the range of high-quality care while adhering to UNOS and CMS regulations. Practices with a variety of specialty-based populations.

Transplant experience is preferred. Functions in collaboration with a physician according to a standard care arrangement and specialty certification.
