Position Summary

This position provides goal-directed support and solution-focused interventions to achieve treatment goals and objectives identified in the client's individualized treatment plan. This position facilitates services that are designed to assist the client with functional deficits and interpersonal and/or environmental barriers associated with their mental illness. This position will be for direct support clients who meet criteria for SED or SPMI diagnosis and require additional one on one support aside from outpatient therapy or case management alone. This position will be a step-up position from Mental Health Assistant and a step below Case Manager. At times this position will provide services to clients who are experiencing a psychiatric crisis and assist with stabilization or connecting to needed resources.
