Admissions Field RN Hospice 

Responsibilities:

  • Monday through Friday 
  • Admissions of Hospice patients within 48 hours as per physician's orders
  • Responsible for providing the initial assessment and admission for patients referred for Hospice Services.
  • Act as a Liaison with referring institutions for transition of care

    • Requirements:

  • A Registered Professional Nurse with current License
  • Hospice agency experience required
  • Knowledge of hospice regulations and qualifications for hospice admission.
  • Associates Degree; Bachelor’s degree preferred
