The LPN Admissions Care Coordinator primary role will be responsible for reviewing newly admitted hospice patient's charts for accuracy and will QA all necessary forms needed for admissions. He/she will follow-up with patients on the NTUC list & provide additional information about hospice services.
Responsible for answering incoming telephone calls from patients/caregivers/medical professionals. Ability to react quickly and calmly, exercise good judgement and make sound decisions during urgent and routine communications. Proficient in receiving, obtaining and relaying accurate information to be disseminated. Provides skilled supportive patient and family care over the telephone in accordance to certification restrictions. May be required to visit patients when needed. Provides care within scope of practice.
