Job Title: Social Worker

Status: Temporary Part Time (18 Months)

Wage: $44.670 – $57.900 per hour

Duties and Responsibilities

The Social Worker will report to the Patient Care Manager, Healthy Aging Program Social Work and work collaboratively with inter‑professional team members and community partners. Responsibilities include providing social work interventions/services such as psychosocial assessments, discharge planning, crisis/trauma/conflict management, individual/group counselling, education, facilitation of case conferences, data collection, and assisting patients and families in adjusting to healthcare challenges.
