Home From Home Care have developed a unique Positive Behavioural Support Team whose role is to support and develop a diverse range of teams in delivering proactive support to complex individuals.
As a PBS Partner, you will be working with a team of four others PBS Partners under the direction of the PBS Manager.
You will be based within a micro-community supporting up to five registered residential services, however there will be an expectation to carousel around all the HFHC’s settings within Lincolnshire and NE Lincs.
Your main aim will be to increase knowledge and skills of the workforce by delivering best practice and sharing your experience with teams. This will be achieved via blended approach of hands on support, undertaking staff and service user observations and creation of Positive Behavioural Support Plans, based upon assessment needs and formulation of behaviours of concern.
You will be responsible for leading and guiding teams, coordinating support and partnership with working with other stakeholders. Effective communication with both operational teams and senior managers will enable us to unify our approach to the delivery of support and maintain our commitment to One Team working.
The preferred candidate will have relevant qualifications – Undergraduate and/or Postgraduate in Health & Social Care Mental Health, Nursing or Behaviour Analysis etc.
Recent experience of working with adults or children with learning disabilities is essential. Full induction and training will be provided.
